Affiliates and PartnersCommonwealth Care Alliance shares unique relationships to two affiliated not-for-profit clinical organizations: Commonwealth Community Care and Commonwealth Care Alliance Clinical Group. These clinical practices provide services to many of Commonwealth Care Alliance's most vulnerable and challenging members. Commonwealth Community Care(formerly Boston's Community Medical Group) This organization is one of the first medical practices established in the nation to provide community-based primary care to adults with severe physical disabilities. Commonwealth Community Care (CCC)* is comprised of physicians, nurse practitioners, physician assistants, behavioral health clinicians and other clinical and administrative and support staff. The staff of the practice are dedicated to delivering quality medical care that will enhance their patients ability to live independently and productively in the community. CCC was recently recognized by the Premier healthcare alliance and its member hospitals for providing comprehensive care to more than 400 patients with severe physical disabilities. Commonwealth Care Alliance Clinical GroupThis organization provides direct care to many of Commonwealth Care Alliance's members in the Boston and Western Massachusetts areas. Commonwealth Care Alliance Clinical Group is comprised of nurse practitioners, nurses, behavioral health clinicians, medical assistants, and other clinical and administrative support staff. These direct care clinicians provide primary care and care coordination services to elders and persons with disabilities across the Commonwealth.
