Statement of what we do

Commonwealth Care Alliance staff Commonwealth Care Alliance strives to achieve the following programmatic goals in all aspects of our work:

  • Create partnerships between those receiving care and those providing and managing care
  • Promote autonomy, independence, and function to the maximal extent possible for all our members
  • Provide responsive, continuous care that effectively reduces secondary medical complications and substitutes support, home and community services for institutional care when appropriate
  • Contain and stabilize medical costs

Mission

Commonwealth Care Alliance’s mission is to provide the best possible care, tailored individually to the needs of all elders as well as to people across the age spectrum with special health care needs throughout the state of Massachusetts , by bringing to scale proven clinical strategies that improve care and manage costs, within a team-based, consumer-directed, prepaid, care delivery program.

Vision

It is our vision to bring people with complex medical and behavioral health needs high quality and personalized care resulting in improved health and better self-management of chronic illness.

History

Commonwealth Care Alliance, Inc. is a not-for-profit care delivery system committed to providing integrated health care and related social support services. Created in 2003, Commonwealth Care Alliance is a “consumer-governed” organization offering a full spectrum of medical and social services for people with complex needs covered under Medicaid and for those “dually eligible” covered by both Medicaid and Medicare, including:

  • older adults (65+)
  • individuals with serious physical, cognitive, or chronic mental illness

Commonwealth Care Alliance is organized as a "consumer-governed care system” to ensure that the empowered consumer voice is built into all of our activities. The fact that the founding partners of Commonwealth Care Alliance are Community Catalyst, Health Care for All (HCFA) and Boston Center for Independent Living (BCIL) attests to this commitment, and thus fundamentally breaks new ground for the role of advocacy. Health Care For All (HCFA) and Boston Center For Independent Living (BCIL) serve as Commonwealth Care Alliance's Corporate Members; in this role these two organizations select Commonwealth Care Alliance's Board of Directors and have established the organizations bylaws.

Clinical System Design

Although the characteristics of the varied populations to be served by Commonwealth Care Alliance are quite different, there are common care system principles that are key to improving care and managing costs. These principles can be summarized as follows:

  • A mission to serve special populations
  • Specialized Clinical care programs
  • Selective primary care networks with expertise in the management of those with special needs
  • Team approach to care management by supporting the primary care clinician with nurse practitioners, nurses, behavioral health practitioners
  • Care Coordination by the primary care team to optimize the management of medical and psychosocial issues and promote stability
  • 24 hours a day, 7 days a week access to care providers
  • Clinical information systems to support the entire network
  • Promotion of enrollee empowerment and participation in care planning
  • Flexible benefit design to provide care in the most appropriate setting, whether community or facility
