Enrollment

Senior Care Options Program (HMO SNP)

If you are interested in our Senior Care Options Program and think you or a family member may qualify, please call our Member Services team at 1-866-610-2273 (TTY 711). We are available 24 hours a day, 7 days a week.

Notes on Enrolling

Once you are enrolled in the Senior Care Options Program, you must retain your current eligibility for Medicare and MassHealth. However, you will receive your Medicare and MassHealth services exclusively through Commonwealth Care Alliance. If you have private health insurance, you may find that you no longer require the additional health insurance coverage. If you decide to cancel it, your family members, doctor, or team may help you make that decision.

Disenrollment

Enrollment in the Senior Care Options Program is completely voluntary. Similarly, if you wish to disenroll at any point in time you may do so. Voluntary disenrollment becomes effective on the first day of the month after the written request is received by Commonwealth Care Alliance. You are required to continue to obtain all services through Commonwealth Care Alliance and our contracted network of providers and facilities until your disenrollment becomes effective. Contact our Member Services team at 1-866-610-2273 (TTY 711). We are available 24 hours a day, 7 days a week.
