What programs do you offer?

We currently offer the Senior Care Options Program (HMO SNP).

Senior Care Options Program

The Senior Care Options Program is a voluntary comprehensive health plan that provides all the services covered under Medicare and MassHealth, and other benefits if determined necessary by the primary care team. Services are coordinated by the primary care team and are accessed through our provider network.

Senior Care Options Program is for people who:

  • Are aged 65 years or older
  • Are a Massachusetts resident living in Essex, Franklin*, Hampden*, Hampshire*, Middlesex*, Norfolk*, Plymouth* or Suffolk county
  • Have Medicare and MassHealth Standard, or just MassHealth Standard alone
