Provider NetworkCommonwealth Care Alliance is dedicated to providing our membership with an extensive network of health care providers that cover the full spectrum of health care needs, from in-patient hospitalizations to help with day to day at home care needs. We believe it is important to recognize the value of community caregivers and to respect the relationship forged in trust between members and their caregivers. In pursuit of this goal, Commonwealth Care Alliance has contracted relationships with a wide variety of providers, encompassing primary care and physician specialty providers, hospital and rehabilitation facilities, behavioral health providers, certified home health care providers, and a variety of organizations that provide services to our members within their communities, such as adult day health providers, chore services, homemaker services, and personal care attendants. Primary CarePrimary care physicians are key to members' well-being and satisfaction, and with this in mind, Commonwealth Care Alliance members are assigned a primary care physician at the time of enrollment. Our care coordination program is based upon a team approach, and to this end, primary care physicians for the Senior Care Options Program are supported with a nurse practitioner, registered nurse or physician assistant, and a geriatric support services coordinator (GSSC), if necessary. This group of health care professionals is called the primary care team (PCT). Together, the PCT is charged with effectively coordinating and delivering all aspects of a Commonwealth Care Alliance member's care. Commonwealth Care Alliance contracts with specific community-based primary care sites (PCS) to accomplish the delivery of primary care services to our membership. A PCS, in most cases a community-based medical care or physician practice, is an organized medical group that has demonstrated experience with and a particular commitment to the populations that Commonwealth Care Alliance serves. It is at these sites that the PCTs are based and operate from. Home visiting primary care services are also an integral part of our primary care formula, and are employed when appropriate to facilitate a member's care. HospitalsCommonwealth Care Alliance contracts with hospitals in the Greater Boston, Lynn/Northshore, and Springfield, Massachusetts areas. Our hospital network allows our members access to everything from the most routine of services such as out-patient laboratory testing services to the most specialized, such as organ transplantation. Specialty Physician NetworkCommonwealth Care Alliance's membership is able to access the specialty physician services available at all of our contracted hospitals, specialties including but not limited to allergy, anesthesiology, audiology, cardiology, dermatology, emergency medicine, family medicine, gastroenterology, gynecology and obstetrics, internal medicine, nephrology, neurology, oncology, ophthalmology, orthopedics and out patient rehabilitation therapies, otolaryngology, podiatry, pulmonology, rheumatology, surgery, and urology. Commonwealth Care Alliance also offers member access to community health care providers such as chiropractors, dentists, durable medical equipment vendors, psychiatrists, licensed social worker services, and vision care providers. Extended Care FacilitiesExtended care facilities are part of Commonwealth Care Alliance's network to support our member's care needs when they are at a lesser intensity than the care that would be sought through admission to an acute care hospital. Commonwealth Care Alliance has rehabilitation hospitals and skilled nursing facilities in our service areas that are able to provide rehab and therapy care needs for most any circumstance. Home CareCommonwealth Care Alliance provider network offers home care services for skilled nursing, physical therapy, occupational therapy, speech therapy, and home health aid via our contracted certified home health agency relationships. Durable Medical EquipmentCommonwealth Care Alliance's durable medical equipment (DME) provider network is capable of accommodating most any member's DME requirements from simple items such as wound care supplies and splints, to more complex items such as specialized wheelchairs and custom orthotics. Behavioral HealthCommonwealth Care Alliance's behavioral health provider network offers both in-patient and out-patient services for our membership, for care needs such as detoxification, emergency services and crisis stabilization, substance abuse, and Psychological and Neuropsychological testing. Community Based ServicesCommonwealth Care Alliance is committed to helping our membership remain as residents in their communities while enjoying the highest level of function, and best possible quality of life.
